FOCUSED SOAP NOTE AND PATIENT CASE PRESENTATION, PART 1

Psychiatric notes are a way to reflect on your practicum experiences and connect them to the didactic learning you gain from your NRNP courses. Focused SOAP notes, such as the ones required in this practicum course, are often used in clinical settings to document patient care.

For this Assignment, you will document information about a patient that you examined during the last three weeks, using the Focused SOAP Note Template provided. You will then use this note to develop and record a case presentation for this patient.

Please see the information provided and fill in the HILIGHTED missing information.

Introduction:

In this assignment, you are required to document information about a patient that you examined during the last three weeks using the Focused SOAP Note Template provided. Additionally, you will develop and record a case presentation for this patient based on the information gathered. The assignment includes several components, including providing diagnostic results, narrowing down the differential diagnoses to a diagnostic impression, reflecting on the case, and formulating a treatment plan.

Answer:

1. Diagnostic results: In order to develop the differential diagnoses, it is important to include any relevant diagnostic results such as labs, X-rays, or other tests. These results provide objective evidence and help support the reasoning behind the diagnostic impression.

2. Diagnostic Impression: The differential diagnoses should be narrowed down to a single diagnostic impression. To do this, you need to explain how and why you ruled out each of the other possible diagnoses. Your rationale should be supported by evidence from the literature, including pertinent positives and negatives for the specific patient case. For this part, you should include a total of three differential diagnoses, with one of them including unspecified psychosis.

3. Reflections: In this section, reflect on the case and discuss whether or not you agree with your preceptor’s assessment and diagnostic impression of the patient, and provide reasons for your agreement or disagreement. Additionally, discuss what you have learned from this case and what you would do differently if presented with a similar case in the future. Furthermore, include a discussion on legal/ethical considerations, social determinants of health, health promotion, and disease prevention, taking into consideration patient factors, PMH, and other risk factors.

4. Case formulation and treatment plan: This section should include a documentation of the diagnostic studies that will be obtained, referrals to other healthcare providers, therapeutic interventions (including psychotherapy and/or psychopharmacology), education, disposition of the patient, and any planned follow-up visits. Each diagnosis or condition identified in the assessment should be addressed in the plan, and the details of the plan should be presented in an orderly manner.

Additionally, in the context of medication, it is important to discuss the risks and benefits, including potential side effects. It is crucial to inform the client not to stop the medication abruptly without discussing it with their healthcare providers. Furthermore, it is essential to discuss the risks of mixing medications with OTC drugs, herbal substances, alcohol, or illegal drugs, and to instruct the client to avoid this practice. Encourage abstinence, and explain how drugs and alcohol can affect mental health, physical health, and sleep architecture.

Initiation of any medication should be mentioned, along with the reason for prescribing it. If therapy services or referrals to specialists are necessary, they should also be documented. It is important to provide emergency numbers to the client, such as Emergency Services and the Client’s Crisis Line, in case they become actively suicidal or homicidal. Furthermore, if relevant, indicate whether you reviewed hospital records, therapist records, or a PMP report.

Lastly, it is essential to mention that time was allowed for questions and answers, and supportive listening was provided. Evaluate the client’s understanding and agreement with the treatment plan as part of the informed consent process. Follow-up with the primary care physician as needed and indicate the need for any specific labs or diagnostic tests and provide reasoning for ordering them, along with any fasting or patient education requirements. Finally, mention the need for a return visit to the clinic, emphasizing the importance of continued treatment to address chronic symptoms, improve functioning, and prevent the need for higher levels of care.
