APA Format 

  • 1-inch margins, double space and 12-point font, Calibri, Times New Roman, or Arial font throughout – font used must be consistent throughout entire plan so choose one to stick with.

  • Section 8: Application to Health Behavior Theory One page
  • Section 9: Evaluation Plan One page 
  • Section 10: REFERENCES One page
  • Section 11: APPENDIX A – Post-Assessment One page 
  • Section 12: APPENDIX B – Timeline One page 
  • Section 13: APPENDIX C – Budget One page
  • Minimum 10 pages not including TITLE or REFERENCE pages.
  • Write in present or past tense to describe current or past data and research, but everything else must be in future tense because this is a plan that you are proposing to do in the future, if funded.
  • Keep data and facts subjective with references, all opinions/objective information must only be included when talking about what your program will do.
  • Avoid using “I” language throughout, instead use “our team” or “our program” where appropriate.
  • Originality score of less than 30% required on final program plan (Turnitin will not be saving draft scores).
  • Avoid using too many quotes as these increase originality scores drastically, or any if possible. Paraphrase as much as possible.
  • At minimum 10 sources used throughout: 6 must be peer-reviewed journal articles, 4 must be data sources (ONLY .gov, .org, .edu or hospital websites will be accepted for data sources).
  • Bibliography and citations must be in APA style & must be recent –preferably published within 5 years.
  • All references must include a yeat, (n.d.) is not acceptable. Find a date on the page or in the URL. If you absolutely cannot find a date check with me before submitting or do not use this reference since you do not know how accurate the information is based on emerging research.
  • All graphics must be cited and include a table or figure # as well as a description of what is being presented through that graphic.
  • Any sources included in the reference page must also be cited in the paper.
  • PROOFREAD for typos, grammatical errors, inappropriate formatting/citations (especially if using any type of generator or AI system), etc.
