6

Abstract

7

This systematic review examines the role of nursing management in enhancing the quality of care and

8

operational efficiency in outpatient departments (OPDs) in Saudi Arabia, focusing on patient

9

satisfaction and the challenges nursing managers face. The study synthesizes findings from various

10

peer-reviewed articles, government reports, and institutional studies to assess the impact of nursing

11

leadership on improving care delivery and operational performance. The review finds that effective

12

nursing management, including proper staffing, resource allocation, and patient-centred communication,

13

significantly enhances patient satisfaction and reduces wait times in OPDs. However, challenges such

14

as staff shortages, high turnover, and cultural integration of expatriate nurses persist. Strategies

15

identified to address these issues include the implementation of standardized care protocols, continuous

16

professional development, and using technology to streamline operations. The study highlights the

17

importance of aligning nursing management practices with Saudi Arabia’s Vision 2030, which aims to

18

modernize healthcare infrastructure and workforce training. The findings contribute to understanding

19

the complexities of nursing leadership in Saudi OPDs and provide actionable recommendations for

20

improving care quality and operational efficiency. Future research should explore the long-term effects

21

of Vision 2030, the integration of new technologies, and the experiences of expatriate nurses in

22

improving nursing management in OPDs.

26

1. Introduction
1.1 Subject Overview

27
28

Healthcare quality in outpatient departments (OPDs) is a critical aspect within the healthcare

29

system that is of broader focus. Outpatient departments act as entry points for medical care needs, so

30

they play an essential role in patient treatment. The expanding healthcare requirements of different

31

population groups compel healthcare facilities to maintain adequate and efficient healthcare delivery

32

systems. Saudi Arabia’s healthcare system has relied heavily on outpatient departments since the

33

government launched Vision 2030 to advance healthcare infrastructure and service delivery (Suleiman

34

& Ming, 2025). Delivering high-quality services in OPDs depends mainly on proper nursing

35

management practices. The daily management of outpatient services depends heavily on nursing

36

managers who lead staff direction while providing prompt, appropriate care to patients. The healthcare

37

facilities in Saudi Arabia encounter various obstacles, such as patient congestion, extended waiting

38

periods, and irregular care standards that diminish patient satisfaction and health system results

39

(Alrasheedi et al., 2019). The demand for healthcare services in Saudi Arabia grows yearly because of

40

population expansion and societal urbanization (Alasiri & Mohammed, 2022). The role of nursing

41

management in resolving these healthcare issues carries extreme importance. A well-organized nursing

42

team enables efficient resource distribution, standardized patient care, and proper training to meet the

43

healthcare needs of the patient population. Research examining how nursing leadership affects

44

outpatient care quality remains scarce throughout Saudi Arabia’s healthcare system.

45

1.2 A Review of Relevant Prior Research

46

Multiple studies have proven nursing management to be fundamental for healthcare quality

47

delivery. Mober et al. (2024) studied nursing environments that received adequate staffing levels and

48

resources to produce improved patient results. The success of nursing leadership practices in outpatient

49

care creates more satisfied patients, shorter wait times, and higher quality care delivery (Mober et al.,

50

2024). Research about nursing management in Saudi Arabia shows that managers encounter difficulties

51

combining administrative work with clinical tasks and handling workers from various cultures. Mutair

52

Restricted – ‫مقيد‬

3 of 18

(2015) reports that the integration of international nursing staff presents a significant problem because

53

cultural and linguistic barriers obstruct teamwork and communication, resulting in short?care delivery

54

(Mutair, 2015). Alluhidan et al. (2020) additionally establish that inconsistent professional growth

55

opportunities for nurses lead to inadequate care quality in Saudi OPDs. Without effective leadership,

56

the nursing staff may not follow evidence-based standardized care protocols, which are the building

57

blocks of care standardization for safety and quality in different healthcare environments (Alluhidan et

58

al., 2020). It is evident from the above findings that limited understanding exists about the challenges

59

of nursing management in Saudi Arabia’s OPDs. However, there is an even more significant gap in

60

research on nursing leadership’s direct impact on care quality, patient satisfaction, and productivity in

61

this area.

62

1.3. Current Research Gap

63

Research on nursing management in healthcare continues to expand, yet Saudi Arabian outpatient

64

care lacks explicit clarity about the effects of nursing leadership. The literature shows limited evidence

65

examining how nursing management resolves overcrowding and staffing issues to enhance patient

66

satisfaction and care quality in OPD settings (Alasiri & Mohammed, 2022). Research on OPD care

67

quality and operational efficiency through nursing leadership remains underexplored despite existing

68

studies on administration and foreign nurse integration (Alsadaan et al., 2023). The available literature

69

does not comprehensively examine Saudi Arabian nursing management factors, including Islamic

70

healthcare practices and the integrating of foreign nurses within the healthcare system. The current state

71

of research does not clarify how these factors transform nursing administration practices and their

72

impact on improvements in outpatient department care delivery.

73

1.4. Research Question and Hypothesis

74

This study addresses an important research question:

75

How does nursing management contribute to delivering superior

quality of care across Saudi Arabia’s

76

outpatient departments?

77

The research will test the following hypotheses after examining them first.

78

Restricted – ‫مقيد‬

4 of 18

H1: Effective nursing management practices lead to better patient care quality and satisfaction within

79

outpatient departments in Saudi Arabia.

80

H2: The quality of care delivered in OPDs suffers because nursing managers experience significant

81

difficulties handling administrative and clinical responsibilities.

82

H3: Optimal operational efficiency in OPDs becomes possible through implementing standardized care

83

protocols, proper staff training, and improved resource allocation, leading to shorter patient waiting

84

times.

85

The research evaluates these three hypotheses to understand the impact of nursing management

86

practices on Saudi OPD patient outcomes, operational efficiency, and wait time reduction.

87

1.5. Objectives of the study

88

This research investigates how nursing management practices contribute to quality

89

improvement in outpatient care services throughout Saudi Arabia. The research examines nursing

90

manager challenges, patient satisfaction effects, and practical operational strategies to enhance

91

outcomes. A systematic review method will be used to analyze secondary data through peer-reviewed

92

articles, government reports, and institutional studies. This study combines previous studies to deliver

93

complete knowledge about the current nursing management practices in OPDs while presenting

94

evidence-based solutions to enhance care quality standards. The research investigates the influence of

95

cultural elements alongside systemic factors, including the integration of expatriate nurses and Islamic

96

healthcare practices on nursing management practices. The research investigates these elements to

97

reveal strategies that enable nursing managers to successfully handle Saudi Arabia’s healthcare

98

environment while improving patient satisfaction and care quality. The research findings will expand

99

knowledge about nursing leadership in Saudi Arabia while providing beneficial information to

100

healthcare administrators, nursing staff, and policymakers. The research presents actionable guidelines

101

to enhance OPD nursing leadership, which aims to generate better patient results and uphold the

102

healthcare development described in Vision 2030.

2. Materials and Methods

106
107

2.1 Research Design

108

The systematic review design served as the research methodology to effectively gather and

109

analyze information from existing literature through organized procedures. The systematic review

110

served as the research design to provide a detailed analysis of existing knowledge about outpatient

111

department (OPD) nursing management in Saudi Arabia. The review method synthesized published

112

research to show patterns, open areas, and commonalities in the literature about nursing management’s

113

impact on patient service quality, satisfaction, and operational efficiency.

114

2.2 Participants/Sample/Study Sample

115

As a systematic review, the research did not require participant involvement or primary data

116

collection. The research drew its data from peer-reviewed articles, government reports, healthcare

117

databases, and institutional studies.

118

Source Population and Target Population:

119

All research articles, government reports, institutional studies, and healthcare databases that

120

examined nursing management in outpatient departments in Saudi Arabia made up the source

121

population for this research. Studies about nursing management practices, operational efficiency,

122

patient satisfaction, staffing levels, and quality of care in OPDs formed the target population.

123

Sample

124

No direct participants included in this systematic review as it did not involve participant

125

interaction. The research used secondary data from peer-reviewed articles , government reports, and

126

healthcare databases as its sample collection. The research utilized all available studies from selected

127

databases to determine its sample size.

128

Sampling Procedure

129

The research used an extensive search approach to retrieve suitable studies from trusted databases,

130

including PubMed, Scopus, and Google Scholar. The studies which met the established research

131

Restricted – ‫مقيد‬

6 of 18

question criteria and inclusion requirements were considered. The assessment of study suitability for

132

review included abstract and full-text evaluation.

133

Inclusion Criteria

134

The research only considered studies investigating nursing leadership roles in outpatient care

135

facilities throughout Saudi Arabia. The research required investigation into staffing levels, resource

136

distribution, patient satisfaction levels, and care quality assessment. The research examined studies

137

published during the last decade because the authors wanted to assess findings that match the present

138

Saudi Arabian healthcare environment. The available research materials had to be in English or Arabic

139

to guarantee accessibility and relevance to the research context.

140

Exclusion Criteria

141

Research studies focusing on inpatient care, non-Saudi healthcare systems and subjects outside

142

nursing management in outpatient settings were eliminated from the review. The study required this

143

exclusion to stay focused on the particular challenges and strategies directly affecting nursing leadership

144

in outpatient departments of Saudi Arabia. Research lacking specific information about nursing

145

leadership and operational efficiency in OPDs received exclusion to focus on pertinent applicable data.

146

Ethical Considerations

147

The significant ethical aspect of this study consisted of correctly citing all data sources. Due to

148

secondary data utilization, the research did not require patient confidentiality procedures or informed

149

consent documentation. The research maintained ethical research standards through proper citation of

150

all used sources.

151

IRB Approval

152

IRB approval was unnecessary since the study collected no primary data and worked with no

153

human participants. The systematic review followed ethical guidelines by providing complete

154

references to all sources and respecting intellectual property rights.

155

2.3 Measures Included in the Research

156

The study adopted measures from the outcomes and variables discovered in the chosen secondary

157

data sources. The research focus variables comprised patient satisfaction, operational efficiency, quality

158

Restricted – ‫مقيد‬

7 of 18

of care, and nursing management practices. The variables were assessed using the information presented

159

in the reviewed studies. The review assessed nursing management practices by evaluating leadership

160

strategies, staff coordination, resource management, and patient satisfaction through survey results or

161

interview responses documented in the studies.

162

When applied to outpatient department nursing management, secondary sources provided data

163

regarding these variables. The data extraction process aimed to discover essential outcomes and

164

challenges alongside nursing managers’ methods to enhance both patient care and operational

165

achievement. The systematic review nature did not require questionnaire implementation as part of the

166

research process. The collected variables depended on data retrieved from established publications. The

167

data analysis focused on qualitative examination of themes, including nursing management practices,

168

patient outcomes, and operational efficiency.

169

2.4 Data Collection

170

Informed Consent

171

Based on existing research data, the study did not require informed consent because secondary

172

data was employed. The research depended entirely on published studies, and its ethical framework

173

appropriately cited all the sources used.

174

Time and Duration of Data Collection

175

The literature review lasted two months, during which researchers performed an organized

176

evaluation of published materials. A literature search took place in February, leading to selection and

177

reviewing of suitable studies in March. The data extraction process, together with thematic analysis,

178

was finished in April.

179

Mode of Data Collection

180

The study used secondary source analysis to obtain data. The research obtained peer-reviewed

181

articles and relevant studies from academic databases, including PubMed, Scopus, and Google Scholar.

182

Data-gathering required researchers to review abstracts and full-text documents to evaluate their

183

connection to the research inquiry.

184

Time Taken to Complete the Response

185

Restricted – ‫مقيد‬

8 of 18

Data collection took an average of one to two hours per study across the duration of the response

186

development. The duration needed to finish the data extraction phase differed according to article length

187

and complexity, the evaluation of abstracts and selection process for studies required about one to two

188

hours for each study.

189

2.5 Data Analysis

190

Data Sifting

191

The relevant studies were selected??, after which the data was sifted to retain only those that

192

fulfilled the established inclusion criteria. The evaluation step focused on studying the quality and

193

appropriateness of each study to determine their significance in understanding nursing management

194

practices in OPDs.

195

Statistical Software

196

The extracted data analysis did not need statistical software since the study was a systematic

197

review. NVivo and similar qualitative data analysis software helped researchers conduct thematic

198

analysis and manage information gathered from multiple studies.

199

Descriptive Statistics

200

The researchers used descriptive statistics to present important findings from their chosen studies.

201

A summary of patient satisfaction metrics, as well as operational efficiency indicators and nursing

202

management strategy outcomes, was provided in the analysis.

203

Statistical Tests

204

This systematic review excluded chi-square tests, correlation analysis, or t-tests because of its

205

nature. This analysis merged qualitative findings from chosen research to find regular patterns in

206

nursing management techniques , patient satisfaction ratings, and healthcare delivery quality standards.

207

Normality Testing

208

Since the research did not collect primary data directly, normality tests became irrelevant. The

209

study’s purpose involved combining results from existing research to discover patterns and themes

210

connected to the research inquiries.

211

3. Results

Restricted – ‫مقيد‬

212

9 of 18

This systematic review presents findings about the nursing management contribution to outpatient

213

department (OPD) care quality in Saudi Arabia by examining managerial challenges , patient

214

satisfaction effects, and operational efficiency enhancement methods. The combined results from the

215

examined studies produced important themes throughout the research questions.

216

Impact of Nursing Management on Patient Satisfaction

217

The review repeatedly demonstrated that nursing management plays a vital role in improving

218

patient satisfaction in outpatient departments. Several studies established that nursing leadership

219

excellence through planned staff distribution, resource organization, and education programs led to

220

higher patient satisfaction outcomes. Patients treated inefficiently managed OPDs expressed greater

221

satisfaction because they received both prompt care and individualized attention, according to Almutairi

222

et al. (2024). The patient satisfaction levels of OPDs declined when they experienced management

223

issues, including insufficient staff and imbalanced resource distribution, according to Alrasheedi et al.

224

(2019).The study conducted by Molina-Mula and Gallo-Estrada (2020) showed that better patient

225

perceptions of care resulted from nursing managers who focused on building strong nurse-patient

226

relationships through communication. Research shows that patient-centred nursing management leads

227

to better clinical care quality and better patient experiences, according to Molina-Mula and Gallo-

228

Estrada (2020).

229

Operational Efficiency and Resource Management

230

Nursing management practices demonstrate essential importance for enhancing the operational

231

efficiency of OPDs. The examination demonstrated strategic management approaches to improve

232

operational effectiveness through proper nurse-to-patient ratios, minimal waiting periods, and resource

233

distribution optimization. Multiple research investigations showed that nursing management

234

approaches directly influence patient waiting periods, thus impacting patient satisfaction levels. For

235

instance, according to Alrasheedi et al. (2019), nursing managers adopting well-designed triage

236

methods followed by high-acuity patient prioritization strategies successfully shortened patient waiting

237

periods, produced operational processes and satisfied patients. The healthcare clinic achieved

238

operational excellence and decreased administrative tasks through nursing management systems that

239

Restricted – ‫مقيد‬

10 of 18

integrated technological scheduling and resource management components (Galiano et al., 2024).

240

Research data shows that nursing leaders can improve operational aspects and patient care delivery in

241

OPDs.

242

Challenges Faced by Nursing Managers in OPDs

243

The analyzed studies documented several obstacles that Saudi Arabian OPD nursing managers

244

encounter while showing positive results from effective nursing management. The main hurdles

245

experienced by nursing managers in Saudi Arabian OPDs consisted of workforce inadequacy, frequent

246

staff replacements, and difficulties integrating foreign nursing staff. Research by Alsadaan et al. (2023)

247

and Mutair (2015) demonstrated that these issues formed obstacles to efficient standardized care

248

protocol execution and operational enhancement. Managing staff from different cultures and languages

249

presented significant challenges to nurse leaders working with many foreign nurses. According to Rosen

250

et al. (2018), cultural differences between nursing staff resulted in broken communication and poorly

251

coordinated teams, ultimately affecting patient care quality (Rosen et al., 2018). According to Alasiri

252

& Mohammed), the healthcare service demand growth in Saudi Arabia has intensified resource

253

limitations and workforce shortages.

254

Strategies for Enhancing Nursing Management in OPDs

255

The reviewed studies presented important findings about operational strategies managers in OPDs

256

should use to enhance quality and operational performance. All studies showed a shared practice of

257

deploying standardized care protocols to deliver uniform treatment experiences to patients visiting any

258

facility. Patient satisfaction increased alongside better clinical results in healthcare facilities that used

259

standardized care protocols, according to Alkorashy and Al-Hothaly (2022). The literature highlighted

260

the continuous professional development of nursing staff as a vital strategy. Staff retention strategies

261

identified as fundamental for preserving top-quality nursing talent included providing professional

262

development opportunities and establishing supportive work environments (Alsadaan et al., 2023).

263

Influence of Vision 2030 on Nursing Management

264

Several investigations analyzed how Saudi Arabia’s Vision 2030 initiative affects nursing

265

management procedures within OPDs. Under Vision 2030, Saudi Arabia dedicated its funds to

266

Restricted – ‫مقيد‬

11 of 18

healthcare infrastructure development, employee education, and technological improvements that

267

modified nursing management practices. According to Alasiri & Mohammed (2022), nursing managers

268

now have the responsibility to deploy new practices and technologies that support the large-scale

269

objectives of Vision 2030 to enhance operational performance and service quality. The implementation

270

of new technologies has created multiple challenges despite the achieved advancements. According to

271

Galiano et al. (2024), the effectiveness of technological tools for operations depends heavily on

272

adequate training and support for nursing staff. Some operational inefficiencies occur in OPDs due to

273

certain areas’ resource and training limitations.

274

A systematic review of nursing management practices in Saudi Arabian outpatient departments

275

(OPDs) provides the essential findings in Table 1. The research demonstrates that strong nursing

276

administration generates benefits for patient contentment, operational excellence, and health service

277

quality yet recognizes workforce problems and cultural obstacles.

4. Discussion
Statement of Principal Finding

280
281

The systematic review established that nursing management is vital for enhancing operational

282

efficiency, quality of care, and patient satisfaction levels throughout outpatient departments (OPDs) in

283

Restricted – ‫مقيد‬

13 of 18

Saudi Arabia. Nursing leadership strategies involving proper staffing activities, resource management,

284

and patient-centred interactions resulted in better patient satisfaction rates and improved clinical results.

285

The review study validated the research hypothesis that quality nursing practice leads to better patient

286

results through shorter waiting periods and advanced care provision. The problems of high employee

287

departure rates, staff deficits, and cultural difficulties for international nursing staff continued to present

288

substantial barriers. The research induced different methods combining standardized treatment

289

guidelines, staff training, and technological implementations to eliminate the identified obstructions

290

while maximizing care excellence.

291

Strengths and Weaknesses of the Study

292

The study’s main advantage comes from its extensive evaluation, which combined various

293

research studies on Saudi OPD nursing management practices. The research design allowed researchers

294

to develop a complete understanding of patient satisfaction and operational efficiency as well as care

295

quality factors. The use of secondary data restricted the study because it depended on the quality and

296

range of included studies. The systematic review methodology effectively found patterns in published

297

research, but original data collection from the field would have provided more direct insights into actual

298

nursing manager challenges.

299

Strengths and Weaknesses of Other Studies

300

The research findings of this study build upon and reinforce previous studies. The review

301

supported previous findings by Almutairi et al. (2024) and Alrasheedi et al. (2019), demonstrating that

302

nursing management positively affects patient satisfaction and operational efficiency. Additional

303

insights emerge from this research because it explores explicitly how Saudi Arabia’s Vision 2030

304

initiatives affect nursing management practices. The infrastructure development and work, workforce

305

and te, and technological enhancements initiated by Vision 2030 have influenced how leaders perform

306

in OPDs. The review examined nursing management practices in OPDs influenced explicitly by the

307

reforms mentioned in Alasiri Mohammed (2022). The research demonstrated significant findings yet

308

revealed that more studies need to be conducted about integrating expatriate nurses alongside cultural

309

challenges in OPD environments. Rosen et al. (2018) addressed these issues, yet additional research is

310

Restricted – ‫مقيد‬

14 of 18

necessary regarding nursing managers’ approaches to bridging cultural gaps for improved team

311

coordination and care delivery.

312

Possible Implications of the Study

313

The research findings create significant implications for healthcare administrators, policymakers,

314

and nursing managers. The study establishes that healthcare administrators and policymakers should

315

dedicate funding to developing nursing leadership programs. Developing the necessary skills for

316

nursing managers in staffing, resource management, and communication would result in better patient

317

care and operational efficiency. Meticulous attention to staffing problems and excessive nursing

318

employee departures is vital in sustaining a dependable workforce with high professional standards.

319

Governmental authorities should establish retention programs with professional development support

320

and workplace support systems for healthcare staff. The research demonstrates why Saudi healthcare

321

needs to incorporate foreign healthcare professionals effectively. Training programs about cultural

322

competence and communication skills should be developed to teach foreign nurses about handling

323

cultural and language differences so they can provide better care to patients.

324

Unanswered Questions

325

The research delivers relevant findings, but additional questions still need clarification. Future

326

research should focus on understanding the extended effects that Vision 2030 will have on nursing

327

management practices within Saudi Arabia. According to this review, additional research is required to

328

determine the lasting effects of Vision 2030 national reforms on nursing leadership. The analysis

329

requires additional research to understand how nursing management affects clinical results that extend

330

past patient satisfaction measures, including treatment success rates, protocol compliance, and recovery

331

patterns. Research lacks insight into how nursing managers embrace modern healthcare technologies

332

such as electronic health records and telemedicine within outpatient department management. Studies

333

addressing technology adoption challenges must continue because researchers require more information

334

about nursing management’s transitional process alongside methods to enhance medical service quality

335

and operational efficiency.

336

Future Research

337

Restricted – ‫مقيد‬

15 of 18

Future investigations need to concentrate on multiple specific research areas. New research should

338

study how nursing management techniques influence healthcare results and healing duration. Research

339

must examine nursing management practices across Saudi Arabian regions by comparing treatment

340

approaches between urban-rural and public-private healthcare facilities. Research needs to investigate

341

how technological progress shaped by artificial intelligence and telemedicine affects nursing

342

management practices in Outpatient Departments. Knowledge of nursing leaders’ technology utilization

343

methods and operational efficiency changes through these tools generates important data for better

344

patient care practices. Research must explore the working experiences of foreign nurses in Saudi OPDs

345

while evaluating different integration approaches meant to improve their workforce integration.

346

Research on cultural competency training and team-building strategies for multicultural settings would

347

allow nursing managers to handle communication issues better and enhance patient care delivery.

348

Final Thoughts/Recommendations

349

This research demonstrates that nursing management is essential in developing superior

350

healthcare quality structures for OPDs throughout Saudi Arabia. Strategic staffing and resource

351

management under effective nursing leadership lead to better operational efficiency and enhanced

352

patient satisfaction, centring on patient care. Healthcare organizations should resolve problems,

353

including workforce deficits, cultural hurdles, and technological implementation challenges. Healthcare

354

administrators and policymakers need to establish nursing leadership development programs while

355

implementing measures to enhance retention rates of healthcare staff and facilitate expatriate nurse

356

integration. Future investigations should analyze the extended impacts of Vision 2030 on the healthcare

357

system, study new technological applications for nursing administration, and understand the working

358

conditions of foreign nurses in Saudi OPDs.

359
360

5. Conclusion

361

The systematic review established nursing management as a critical factor that influences quality

362

of care delivery and OP, operations and patient satisfaction within Saudi Arabia’s outpatient

363

departments. Patients experience better care outcomes and satisfaction when nursing leaders implement

effective leadership strategies, including staffing practices, resources on decisions and PA?, and client-

365

facing communication techniques. The research results confirm that properly managed outpatient

366

departments benefit from nursing leadership’s dedication to strategic planning and resource

367

management, which generates superior clinical results and enhanced patient experiences. The review

368

exposed multiple ongoing difficulties, including extensive nurse staff turnover, insufficient staffing,

369

and cultural obstacles when attempting to integrate foreign nurses. Multiple organizational obstacles

370

prevent nursing management strategies from being fully deployed while blocking the creation of

371

uniform quality care delivery. Throughout the review, the authors presented effective solutions,

372

including standardized care protocols, continuous professional development and technological

373

integration, which help improve nursing management despite identified molecules?. The study results

374

generate significant consequences for healthcare administrative and policy development teams. The

375

healthcare system requires ongoing funding to train nursing leaders and develop staff retention

376

approaches and cultural competency initiatives to solve staffing shortages and enhance teamwork. The

377

implementing technologies requires sufficient training for nursing managers to maximize their

378

usefulness in improving operational efficiency Advancement of nursing management in Saudi OPDs

379

has shown positive results, but additional efforts are required to eliminate all barriers discovered in this

380

research. Govt research must continue to measure how Vision 2030 influences nursing leadership and

381

how new technologies with cultural integration strengthen OPD care delivery.
